Schedule an Appointment

New passport and renewal applications are accepted by our office Monday through Friday from 9 am to 3 pm BY APPOINTMENT ONLY.

Processing Times

Current estimated processing time to receive your passport from when the passport agency receives your application:

  • 6-8 weeks for routine service
  • 2-3 weeks for expedited service
Note: The National Passport Office recommends applying 6 months ahead of the travel date.

New Applicants Form DS-11

First-time applicants, minors applying with two parents/guardians and applicants applying first time as an adult must fill out an application DS-11 eforms.state.gov/Forms/ds11.pdf or travel.state.gov/content/travel/en/passports/how-apply/forms.html and print it single sided before the appointment.

Applicants can also stop by our office to pick up the application Monday - Friday from 9 am to 5 pm.
NOTE: The application must be filled out in BLACK INK only. If an error is made you must complete a new form. Do not correct!

Each application must be accompanied by:

  1. application DS-11;
  2. proof of U.S. citizenship as an Original Birth Certificate, Certificate of Naturalization, previous passport, or other original documents as required;
  3. proof of identity as valid state driver’s license, state ID;
  4. one recent passport picture (2″x2″ in size);
  5. appropriate fees – one check or money order per application, payable to “U.S. Department of State”. The fees are standardized by the U.S. Federal Government for all passport applications, thus will be the same in Maine Township as any processing location.

Renewal Applicants Form DS-82

Passports are eligible for renewal if the passport is a 10-year adult passport and it was issued less than 15 years ago.

Documents required:

  1. application DS-82
  2. one recent passport picture (2″x2″ in size);
  3. appropriate fees – one check or money order per application, payable to “U.S. Department of State”;
  4. most recent passport;
  5. original marriage certificate or court-ordered name change document (if applicable).
All passport renewal applications are completed by mail by the applicant
and mailed through the United States Postal service (USPS) with the address provided on the application.
